Maximum Healing Spell Ranks indicate the maximum rank of each spell that will ever be cast. If ever the rank of spell is determined to be above what is set here, the rank will be reduced to the maximum set here.

Required Minimum is a very dangerous setting. If the rank of the spell is determined to be under the minimum value set here, no spell will be cast. Only if the minimum determined is equal to or greater than the setting here will a spell be cast. If you want to ensure that you always cast a spell at a minimum level (assuming recipient can handle it and you have enough mana) use the overhealing ranks instead.

Minimum (Raised) will ensure that the spell that is cast will always be at this minimum level (assuming there is enough mana, etc.).
